The myocardial and coronary histopathology and pathogenesis of hypoplastic left heart syndrome.

چکیده
Hypoplastic left heart syndrome has the greatest mortality rate among all CHDs and without palliation is uniformly fatal. Despite noble efforts, the aetiology of this syndrome is unknown and a cure remains elusive. The genetic and anatomic heterogeneity of hypoplastic left heart syndrome supports a rethinking of old hypotheses and warrants further investigation into the histological and vascular variations recognised with this syndrome. In an effort to elucidate the pathogenesis of hypoplastic left heart syndrome, this review will focus on its unique myocardial and coronary pathology as well as evaluate the association of hypoplastic left heart syndrome with the endocardial fibroelastosis reaction.

Acute myocardial infarction as a cause of death in palliated hypoplastic left heart syndrome.
A 20 month old child with hypoplastic left heart syndrome died suddenly from a massive myocardial infarction 15 months after a hemi-Fontan operation. This was confirmed at postmortem examination and histological examinations. The sites of surgical reconstruction were all in good condition, there were no gross anatomical coronary abnormalities, and the coronary ostia were unobstructed. On micros...
متن کاملThe Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery (ALCAPA): a Case Series and Brief Review
Background Anomalous left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital cardiovascular defect that occurs in approximately 1/300 000 live births or 0.5% of children with congenital heart disease. There are two types of ALCAPA syndrome: the infant type and the adult type. The most infants experience myocardial infarction and congestive heart failure, and approximately 9...
متن کاملAnomalous origin of circumflex coronary artery from right pulmonary artery in a hypoplastic left heart syndrome child
We describe the case of a newborn with hypoplastic left heart syndrome (HLHS) and aberrant origin of the circumflex coronary artery from the right pulmonary artery. The patient underwent a modified Norwood procedure with direct reimplantation of the circumflex coronary artery to the neo-aorta, but died on the 5(th) postoperative day because of myocardial failure. Detailed assessment of coronary...
متن کاملComplement factors in Acute Myocardial Infarction and Unstable Angina
Background: Coronary artery disease (CAD) is one of the most important and lethal diseases in the world. CAD represents a board spectrum of disease from silent ischemia at one end to sudden cardiac death at the other end. The middle of this spectrum consists of acute myocardial infarction (AMI) and unstable angina pectoris (UA). Recent data show that the inflammatory process plays a major r...
متن کاملMitral Stenosis and Aortic Atresia--A Risk Factor for Mortality After the Modified Norwood Operation in Hypoplastic Left Heart Syndrome.
BACKGROUND There are conflicting reports regarding the importance of mitral stenosis and aortic atresia as a risk factor for Norwood mortality. This study reviews outcomes of this anatomic subgroup at our institution and examines the utility of preoperative cardiac catheterization and its correlation with clinical outcomes and pathology findings. METHODS This is a single-center, retrospective...
